The Icelandic Red Cross was founded around ambulance services in 1924. A contract with the welfare ministry was signed in 1997 to ensure safety and responsibility in operations.

Transportation fees are decided by the Icelandic authorities.
For individuals covered by health insurance: Individual transferred with an ambulance and is covered with health insurance in Iceland pays a fee of 9.871 ISK (valid from 1.1.2025). Retirement pensioners aged 70 and over pay a lower fee, or 7.403 ISK. People on disability benefits (60% or more) get 50% discount, but they must apply for it separately to the Red Cross for the first time and present a disability card from the National Insurance Institute.
For individuals not covered by health insurance: For ambulance transport, the patient shall pay a fee of ISK 59,577 for the Ambulance Crew (Service charge not billed by the Red Cross) for each hour started, plus ISK 3,774 per kilometre driven up to and including 65 kilometres, but a minimum of 15 kilometres and ISK 875 per kilometre driven in excess of 65 kilometres. Registration is coordinated by 112 according to an official decision. 112 registers all transfers and sends a notification electronically to the Red Cross. Information that are sent are; name, locations, amount and address, details on reason for transfer are not included.

Special fund runs the project called "Ambulance fund". The Icelandic Red Cross is responsible for all ambulances in Iceland according to a contract with SÍ.
The Icelandic Red Cross co-operates with the Icelandic authorities and Healthcare institutions regarding ambulance services in Iceland. As decided by Icelandic authorities, ambulance and medical services are billed seperately. What you already paid for is the service you received in the ambulance and/or at the Healthcare Institution. The invoice sent to you by the Icelandic Red Cross is only for the ambulance transport itself.
If more than one person were transported and treated or examined at the Healthcare Institution, all individuals qualify for an ambulance transport according to Icelandic Regulations no. 1577/2024. Under the aforementioned regulation, a case is classified as an ambulance transfer when an individual is transported to a healthcare facility for examination or treatment. Therefore, confirmation of hospital evaluation—regardless of the level of treatment—qualifies the transport under the regulated ambulance fee structure.
